Anyone can make a referral to Delta Community Support. Whether you are an NDIS participant looking for a new provider, a family member or carer seeking services on behalf of a loved one, a support coordinator managing a participant's plan, a plan manager, or a health professional working with someone who could benefit from our services — we welcome your referral.
All referrals are treated with the utmost confidentiality and handled by our experienced intake team. We understand that choosing a disability support provider is an important decision, and we are committed to making the process as simple and transparent as possible. Once we receive your referral, our team will review the details and respond within 24 hours to discuss next steps.
